The Montfort Ottawa West Community Family Health Team will soon provide primary health care services in French at the Maison de la francophonie d’Ottawa.
Our mission is simple: to improve access to high-quality French-language health care for Francophones living in Ottawa West and to help build healthier communities. Thanks to an investment from the Ontario Ministry of Health, the team will gradually take on up to 6,725 patients by the end of 2027.
Located in a growing Francophone community, the clinic will bring together an interprofessional team committed to providing accessible, compassionate, and collaborative care.
Patients
Looking for primary care provider?
Patient enrollment will take place gradually over the coming months. Individuals who wish to eventually receive care from our team are encouraged to register with Health Care Connect, Ontario’s provincial program. We will first prioritize Francophones who do not currently have a primary care provider and who live in neighbourhoods with postal codes beginning with K2, or K1R, K1S, K1T, K1V, K1Y, and K1Z.
